We are an IT Firm looking for a driven, talented, creative, and skilled PHP Developer to work with us to develop a high-traffic web-based assessment tool. We are looking for an experienced developer with an impressive portfolio and referemces, which we will be checking
We will be working together on planning and create lists of deliverables, which will be provided by the Developer.
Specifics:

- Design an online form and interface to capture information about the customer, as well as algorithms to evaluate the customers' strengths and weaknesses by means of a "ratings scorecard" using a paper based assessment tool. We have developed a
preliminary paper based assessment tool and will need the selected vendor to take the paper assessment and update/change/revamp it for the online assessment tool.
- Host these interactive forms in a web application so that businesses can enter their information in a secure web area and monitor the progress of their application
- Create a dashboard for us to monitor the usage of the assessment, manage, access, and download reports in XLS format. We will use this dashboard to monitor and track the progress of its customers.
- Integrate the outputs of this application with our existing application, such as Salesforce.
